Lottery Conditions of the Glenn Index Conflict Health® Questionnaire

        * The purpose of the lottery is to encourage eligible employees to complete the Glenn Index of Conflict Health® Questionnaire (or "GICH Questionnaire").

        * The Winner of each week's GICH Questionnaire Lottery draw will win 2 Adult Movie Ticket vouchers valued at approximately $36 retail.

        * To be eligible to enter the GICH Questionnaire Lottery, entrants must: (i) be a current employee of a participating client organisation; (ii) be invited to complete that week's CHQ; and (iii) complete the CHQ by 8.00pm AEST Friday of that week.

        * The closing time and date of the GICH Questionnaire Lottery is 8.00pm AEST each Friday during the period July 1 to December 9, 2011.

        * The GICH Questionnaire Lottery will be drawn by James Glenn at the offices of Conflict Health at 5 Somerset Street, Richmond in Victoria on the following Monday of each week.

        * The Winner of each week's draw will be advised by return email and their name and worksite location posted, as shown above, on this webpage: http://www.glennindexofconflicthealth.com/conflicthealthquestionnaireconditions.html

Winners' details will be updated on this webpage by 5.00pm AEST on the following Monday of each week.

        * Only entrants from the current week's GICH Questionnaire are entered into the lottery draw to win i.e. previous week's entries are excluded from re-entering later week's lottery draws.

        * Once the GICH Questionnaire Lottery closes for the week, no further entries will be accepted into the draw for that week.

        * Each entrant has an equal chance of winning the GICH Questionnaire Lottery.

        * Prizes are not exchangeable for cash or other products or services.
